Angels of Mercy Home Healthcare Services ("we," "us," or "our") is committed to protecting the privacy of the families, participants, referral partners, and caregiver applicants who interact with our website and services. This page is maintained by Angels of Mercy Home Healthcare Services and explains, in plain language, what information we collect, how we use it, and the choices you have. It is not a substitute for legal advice or for the full HIPAA Notice of Privacy Practices provided at the start of care.
Information we collect
- Care & referral requests — name, phone, email, relationship to the participant, and the message you send through our forms.
- Caregiver applications — name, contact details, qualification (CNA, GNA, HHA, etc.), license or certification number, area of residence, experience, and any optional notes.
- Protected Health Information (PHI) — when we provide care, we collect health, demographic, and service information needed to deliver and coordinate care.
- Site usage data — limited technical information (e.g., browser type, pages viewed) used to keep the site working and secure.
How we use information
- Respond to care, referral, and employment inquiries.
- Provide, coordinate, and document home care services.
- Comply with Maryland and federal health-care regulations.
- Protect the safety of participants, families, and staff.
We do not sell your personal information. We do not use form submissions for marketing without your consent.
How we protect information
Form submissions are stored in access-controlled systems and transmitted over encrypted connections. Access to PHI is limited to authorized staff with a legitimate need to know, consistent with HIPAA's minimum-necessary standard.
Your rights under HIPAA
If you are a participant in our care, you have the right to: inspect and request a copy of your record; request corrections; request restrictions on certain uses and disclosures; request confidential communications; and receive an accounting of certain disclosures. Angels of Mercy Home Healthcare Services provides services without regard to race, color, national origin, religion, sex, age, disability, sexual orientation, gender identity, or source of payment.
